To get you ready to go, we need three things. Use the links below for each item. Feel free to bookmark this page for later reference.

Please complete these no later than 48 hours prior to your first appointment.

1. Schedule your appointment(s) (one at a time or all at once).

2. Complete a couple of setup forms .

3. Setup a Payment Schedule.

PLEASE NOTE: We have a 24 cancellation policy. When you schedule, we take your credit card information, but ONLY charge it if you are a “no- show” or cancel less than 24 hours before the appointment.

If this is a “free” appointment and you are a “now show”, then the “free” appointment will no longer be available to you in the future.
