This form is required for employees. It is an authorization from the employer to Fire Mountain Consulting. It grants us permission to provide and bill for Life Coaching services.

Parent or Guardian Authorization

This form is required of Life Coaching clients who are minors or wards of a parent or guardian. It grants us permission to provide Life Coaching services.
